Paulette Schwartz, LCSW, CPXP
Director of Human Centered Care, Hartford HealthCare
Paulette Schwartz, LCSW, CPXP is the Director of Human Centered Care for Hartford HealthCare’s Community Network. Her daily work consists of teaching and coaching best practices to improve the patient and colleague experience. She works directly with Hartford HealthCare’s Rehabilitation Network, Senior Services Skilled Nursing and Assisted Living facilities, Special Needs programs and Homecare division which includes certified, hospice and private pay levels of care. In her role, she partners with the executive leadership teams and all levels of staff.
Paulette worked with the Hartford HealthCare at Home Hospice team for 5 years before joining the Office of Experience. Prior to HHC, she was the Director of the Cancer Services Program for Suffolk County in NY and previously spent over 10 years in the field of Alzheimer’s and dementia. Paulette is passionate about creating environments where humanistic interactions are a priority of life. She earned her Master of Social Work in 2002 from Fordham University and will be completing her Master of Public Health from Southern CT State University in the fall.
