Jenny Azzara, MM is the Senior Director, Performance Improvement & Organizational Development at Community Care Cooperative (C3), a MassHealth Accountable Care Organization. In this role, Jenny is responsible for designing and implementing initiatives with C3’s participating health centers to facilitate efforts to achieve goals for quality care and total cost of care. In addition to her position at C3, Jenny serves at the Senior Practice Transformation Specialist at the HMS Center for Primary Care.
Prior to joining C3, Jenny spent several years in practice management and quality improvement roles in Boston-area health care systems and launched a consulting practice in 2013. Jenny holds a Bachelor of Arts in Psychology from Boston College and a Master of Health Policy and Management from the Heller School at Brandeis University.
